Case study:

Jonathan was referred to our Diabetes Management Program by his GP under the Group Allied Health Service for Patients with Type 2 Diabetes through Medicare. His referral included 8 group sessions in addition to an initial consultation and follow up consultation.

In the initial consultation Jonathan spoke about having Type 2 Diabetes for over 15 years, borderline high blood pressure (148/82), and that he was now insulin dependent. Jonathan’s HbA1c levels provided by his doctor indicated that his blood glucose control was poor, he indicated that he had never participated in formal exercise, and he regularly experienced the feeling of low blood glucose levels. Jonathan stated that he would experience the feeling of low blood glucose levels at least once per week.

Together with Jonathan goals were set which included increasing his muscle mass and improving his long term glucose control.

Jonathan successfully participated in 8 group exercise and education sessions with a particular focus on education regarding his diet. During this time Jonathan kept a food diary and a blood glucose record sheet.

At the conclusion of the Diabetes Management Program Jonathan made significant improvements in his dietary intake. An eating plan was also devised to ensure he did not experience the feeling of low blood glucose levels. Jonathan’s blood pressure improved, 138/69, and he stabilised his blood glucose readings, improving his HbA1c levels to fair (his best ever result)! Jonathan also increased his muscle mass by over 1kg during this time.

Jonathan has opted to continue in our 8- week Diabetes Management Program and has accompanied this with a home exercise program involving resistance training as well as playing tennis 3 times per week.
